当院の産婦人科小田切幸平先生が、5月上旬に
カンボジアの医療施設へボランティア勤務を行ってきました。

ボランティア勤務をした施設は、奄美群島のグループ病院も
支援頂いている吉岡秀人先生（ジャパンハート最高顧問：創設者）が
中心となり立ち上げたジャパンハートの施設です。

ゴールデンウィーク中に、カンボジアで活動している国際医療協力団体
ジャパンハートのこども医療センターに長男、長女を連れてボランティア
勤務してきました。私がお役に立てることはあまりないかと思っていましたが、
そんなことを考える余裕がないくらい、いろんな患者さんを診て、たくさんの経験を
させてもらいました。

まずはカンボジアの歴史を知るため、ポルポト政権時代のトゥールスレン虐殺博物館、
キリングフィールドの見学。その後、病院では腎盂腎炎・水腎症の妊婦や粘膜下筋腫・
過多月経など各種急患の対応。吸引分娩4回施行後の新生児仮死の蘇生。
産後大量出血、重度膣壁裂傷の対応。胞状奇胎、大量出血の処置。
病院での妊婦健診や地域ヘルスセンター(診療所みたいなところ)での出張妊婦健診。
現地カンボジア人スタッフへの産科救急レクチャーやシミュレーション訓練。
当院のiCTGやシミュレーション訓練などの取り組みの紹介講演等々。

長男・長女は、小児がん病棟の子供たちの相手をしたり、外来業務や分娩業務を手伝ったり、
実働部隊として関わらせていただきました。

いろんなことを学びましたが、特に普段当たり前のようにある医療器具や薬剤がない中で、
他国のスタッフと一緒に医療活動をすることは想像以上に大変でした。同時に当院で一緒に働く
スタッフや環境のありがたさを身に沁みて感じました。そして分娩に対する考え方も国によって
違うのだと改めて認識させられました。

今後も奄美の産婦人科医療とともに、こうした活動にも関わっていきたいです。
こうした経験を与えてくださったジャパンハートの皆様、留守を預かってくれた当院スタッフに
心より感謝しております。

During the Golden Week holidays, I took my eldest son and daughter to work as volunteers at the Children's Medical Center of Japan Heart, an international medical cooperation organization working in Cambodia. I thought there was not much I could do to help them, but I saw so many different patients and had so many experiences that I did not have time to think about that.

First, we visited the Toul Sleng Massacre Museum and the Killing Fields during the Pol Pot regime to learn about the history of Cambodia. Then, at the hospital, we treated various emergencies such as pregnant women with pyelonephritis and hydronephrosis, submucosal myoma, and excessive menstruation. Resuscitation of the newborn baby after vacuum delivery. Responding to massive postpartum hemorrhage and severe laceration of the vaginal wall. Treatment of hydatidiform mole and massive hemorrhage. Pregnancy checkups at a hospital and at a community health center (like a clinic). Lectures and simulation training on obstetric emergencies for local Cambodian staff. Lectures on our hospital's mobile iCTG, simulation training, and other initiatives, etc.

My eldest son and daughter were involved in the actual operations, working with children in the pediatric oncology ward and assisting with outpatient services and deliveries.

I learned many things, but it was more difficult than I had imagined to work together with staff from other countries on medical activities, especially in the absence of medical equipment and drugs that are usually taken for granted. At the same time, I was deeply grateful for the staff and environment in which we work together at the hospital in Japan. I was also made aware once again that the way of thinking about childbirth differs from country to country.

I would like to continue to be involved in such experiences as well as obstetric and gynecological care in Amami. I would like to express my sincere gratitude to everyone at Japan Heart for giving me this valuable experience, and to the staff of my hospital for taking care of me while I was away.
